Who is Ba'alzamon? Is he Ishamael or the Dark One or someone else?

Ba'alzamon is Ishamael. Ba'alzamon means "Heart of the Dark" in the Trolloc tongue and most people believe it is their name for the Dark One. In reality, Ishamael uses that name for reasons of his own in the first three books. We have never actually seen the Dark One.
